The Phillipsburg School District does not sponsor this event and the District assumes no responsibility for it. In consideration of the privilege to distribute materials, the Phillipsburg School District shall be held harmless from any cause of action arising out of the distribution of these materials.

Waiver of Liability This agreement releases Town of Phillipsburg from all liability relating to injuries that may occur during any/all activity. By signing this agreement, I agree to hold Town of Phillipsburg entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. I also acknowledge the risks involved in participating and am participating voluntarily. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity. By signing below I forfeit all right to bring a suit against Town of Phillipsburg for any reason. In return, I will be able to participate in all activities. I will also obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed. Photo Waiver I hereby grant the Town of Phillipsburg permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of the Town of Phillipsburg and will not be returned or sold to anyone else. I hereby irrevocably authorize the Town of Phillipsburg to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge the Town of Phillipsburg from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. COVID Waiver COVID-19: The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person to person contact. It is believed that an individual can be infected with COVID-19 without their knowledge and be asymptomatic. The Town of Phillipsburg has put in place preventative measures to reduce the spread of COVID-19. However, The Town of Phillipsburg cannot guarantee that I/We or anyone else will not become infected with COVID-19, including my spouse, guests, unborn child or relatives. Participation in a The Town of Phillipsburg sponsored athletic sports program (s), related activity or event or using The Town of Phillipsburg facilities, could increase the risk of contracting COVID-19. By signing this agreement I/We acknowledge the contagious nature of COVID-19 and VOLUNTARILY assume the risk that I/We may be exposed to or infected by COVID-19 by participating in a The Town of Phillipsburg athletic sports program(s), related activity or event or by the use of The Town of Phillipsburg facilities and that such exposure or infection may result in personal injury, illness, permanent disability, and death to myself, myself or others, including but not limited to, The Town of Phillipsburg employees, volunteers and program participants. I/We understand and voluntarily accept and assume all the foregoing risks related to COVID-19 and accept sole responsibility for any injury or illness that may occur. Further I/We understand and agree that this release includes any Claims based on the actions, omissions, or negligence of The Town of Phillipsburg, its employees, agents, officers, volunteers and assigns, whether a COVID-19 infection occurs before, during, or after participation in any The Town of Phillipsburg athletic sports program(s), related event or activity or by the use of facilities.
